This may be a timing issue.  There's an old instrument parser bug that has a
problem with the output queue if the next input is received before the
output queue has been completely cleaned up - usually about 10 milliseconds
after the computer receives the (instrument output, computer input).  (In
all the other cases I've seen the error is different than the one you
described, but counters can be weird anyway.)

To fix the problem, where ever you have READ..WRITE, insert a WAIT 0.01
   READ ....
   WAIT 0.01
   WRITE ...
