    Unlikely that it's Windows 7 x64.  I just tested this on Win7 x64, MPLAB X v4.15 and the new header created and added to project without issue.  Non-harmony project.  So either something else is causing it (harmony maybe?), or 4.15 fixed it.

    I also doubt that it's just the windows version.  
If you suffer from the problem, then I would
 
a) create a small project with the "New Project" wizard.
   (possibly close your working project before, to have "clean" conditions.
   I did my check with a "standalone project".
   I also took care that the correct project folder is selected BEFORE I use the "New ... " function .
   I did this for both a c. and an .h file  (successfully)
b) If this still does not work, consider Uninstall 4.15 and reinstall, do not reuse "settings from previous version".
 
Since you said, you can manually add the existing file, I assume there is no need to check whether your project.xml is not writeable or in use by some other thread/process.

I have had this Zombie plague for ages.
 
It's not just add, it's also rename, move, remove and delete that you have to do twice.
I found to keep a file permanent was to: restart mplab x, add the file or rename etc...
Do that before any editing otherwise you are back to square one.

On restart mplabx complains that the file was not found, (it is still looking for the original)
and it is but there grayed out.
1. Delete that file since it does not even exist.
2. Again, add existing file.

I've just gotten used to it, it's just another mplabx bug.

    Just tried with a new Standalone Project. The first file suffered the bug, from the second onwards everything went fine.
Then I tried to get what rodims meant with "select correct folder BEFORE using the 'New ...' function" and deliberately selected "Libraries". Then from the menu File -> New File and followed the dialogue. That re-produced the bug. File was created but not included in the project tree.
 
Usually, I right-click on "Source Files" or the like in the project tree to create new files. That seems to work at least from the second file onwards.
